To order a personalized flag, please forward a completed order form and payment to: Honorable Jerry F. Costello, 144 Lincoln Place Court, Suite 4, Belleville, Illinois 62221. Please make your check or money order payable to "Congressman Costello's Office Supply Fund."
Due to high volume at the House Flag Office, personalized flag requests will take 8 weeks to process, once an order is received in my Washington office. To personalize the order, please include the full name of the person for whom the flag is to be flown. If the flag is for a birthday or anniversary, please include the date and the age/anniversary. If the flag is for a military member, please include rank, branch of service, and number of years served. Along with complete information on the recipient of the flag, please be sure to include your name, address, and daytime phone number along with the order.
