If you choose to have your flag flown over the United States Capitol, please complete the following information for the certificate.  Please give our office at least 30 days advance notice for those flags that need to be flown on a particular day (to commemorate a birthday, retirement, or other occasion).  This information will go on a certificate that verifies the authenticity of your flag.

If you have multiple flags to fly on the same date, and you would like different names and/or occasions to appear on each certificate, please leave the name and occasion information below blank.  After clicking on the "Calculate Prices" button below, you will be prompted to use a separate document for the name and occasion.
