| This project addresses the federal focus on affordable and quality health care for all individuals, particularly the need to use prevention to decrease the incidence and severity of serious conditions and diseases and in so doing increase the national health while reducing health care costs. The pilot program planned for the Essex County Hospital Center can provide effective care to prevent the development of Type 2 Diabetes in the severely mentally ill who are at proven high risk by working closely with community health care workers. | |||
