I encourage active duty military personnel to make every effort to resolve an issue through their proper chain of command. Who can help me regarding my TRICARE benefits?

How can I change the character of my discharge or re-entry code?

  • If you are a former member of the Armed Services, click here for instructions and a DD Form 293 application to be submitted to the appropriate review board.
  • If you were released by the decision of a General Court Martial, or if 15 or more years have lapsed since your discharge, you must submit DD Form 149 to the Board for Correction of Military Record for your branch of service.
    • If you served in the Army, mail your completed  DD Form 293 to:

How can I get a copy of my discharge papers, military service records, and/or medals?

I am more than happy to work with the National Personnel Records Center (NPRC)  on your behalf. Please complete the Privacy Authorization Form and mail it to one of my district offices. Be sure to include your full name, social security number, serial number, branch of service, dates of service, date of birth, and unit or battalion assignment. If you have your discharge papers and you are requesting records and/or medals, please include your DD Form 214 to expedite the process.
