Academy Nomination Information

(Please read carefully)

Consideration for nominations will be given to legal residents of Arizona who reach the age of seventeen, but do not exceed that of twenty-two prior to July 1st of the year in which they seek entrance. Files must be completed by October 31. The following items are required for your file:

  1. Application for nomination (PDF) If you don't have the Acrobat reader, Download Acrobat Reader from Adobe.com.
  2. High School Transcript (must be sent by High School)
  3. Three letters of recommendation (one should be from a teacher, counselor or the principal of your school) Please follow up on letters of recommendation to see if my office has received them.
  4. A recent photograph (your picture is important to help me associate you with your file)
  5. SAT/ACT Scores. Be sure that a copy of the scores is mailed to the academy and to my Prescott Office. You can have the testing service send your scores directly to my office by entering the identification number 7224 for the ACT and 4643 for the SAT in the appropriate fields. I cannot emphasize enough the importance of taking these tests early. Not having any test scores in your file could prevent you from being considered for an interview with my committee and myself. It is your responsibility to arrange for and take college entrance exams.
  6. If you are a member of the National Honor Society, a copy of your certificate is required for your file.
  7. Please be sure you have opened a file at one or more of the academies, this is your responsibility.

Nominations to the service academies will be made on a fully competitive basis. For each opening that is available to me, I will nominate up to ten applicants. The nomination of these nominees will be determined primarily by high school scholastic records, ACT/SAT test scores and a personal interview with my committee. Qualified candidates will be notified at least ten days prior to the interview date.

Each academy will give equal consideration to the nominees following the receipt and evaluation of information derived from college entrance examination, physical examinations and a review of high school records, and will make the final selections and appointments.

Although you are applying for a nomination to one of the military academies, you should, at the same time, pursue an alternative goal and apply for admission to the college of your choice.

You might also check into various ROTC programs available at many colleges and universities.

I suggest that you contact my office prior to the October 31 deadline to see if the required material requested has reached my office. I will notify you of your nomination as soon after the interview as possible.

Please mail all information for your file to my Prescott Office at the address below. If you have any questions, call my office at (928) 708-9120 and ask for Don Packard. Thank you.
