Can Congressman Shays assist me in
obtaining my records/medals or those of my family member?
Congressman Shays'
office remains available to assist in obtaining your records
or medals. Veterans or their next-of-kin may request issuance
or replacement of records, as well as military medals and
awards. Family members may only request medals and awards
of living veterans by obtaining their signed authorizations.
For deceased veterans, requests will be accepted from next-of-kin
(unmarried widow or widower, son or daughter, father or mother,
brother or sister of the deceased veteran).
How do I request assistance in obtaining
records/medals?
If you desire assistance in obtaining military records or
awards, please fax
or mail a request for assistance directly to Congressman Shays'
Bridgeport office. In your correspondence, please
include as much of the following information as possible:
- Full Name of Veteran
- Date of Birth
- Place of Birth
- Branch of Service (and unit if possible)
- Service Number
- Social Security Number
- Dates of Service
Remember a signature by the veteran or the next-of-kin (unmarried
widow or widower, son or daughter, father or mother, brother
or sister of the deceased veteran) is required on all requests.
