UPDATE APPLICATION FOR A NOMINATION TO U. S. SERVICE ACADEMY

APPLICATION UPDATE Candidate...If you have already submitted an application and wish to send additional information, e.g., new SAT/ACT scores, change in the order of your choices of Military Academies, recent awards/accomplishments, etc., or if you have decided that you no longer wish to be considered for a nomination (wish to withdraw), type your information below and click on SUBMIT.

PRIVACY ACT STATEMENT: Submission of the requested information constitutes authorization to release it to Congressman Visclosky, his academy advisory committee, and the academy admissions offices.

ACADEMY PREFERENCE: Be sure to fill out an application with the Admissions Office of each of the academies. It is strongly urged that you make alternate plans should you not receive a nomination / appointment.
